Your Key Responsibilities

Direct Client Support

Engage clients in meaningful community activities and social connections

Provide assistance with domestic tasks including light cleaning, cooking, and meal preparation

Offer safe, reliable transport to appointments and activities

Facilitate uplifting, trauma-informed conversations that promote healing and growth

Professional Documentation & Collaboration

Maintain accurate, detailed case notes and progress documentation

Prepare incident reports when required

Collect and implement data for allied health professionals

Follow individualised Case Management and Behavioural Support Plans

Collaborate effectively within our multidisciplinary team including clinical staff, house managers, families, and guardians

Goal Achievement & Advocacy

Support participants in working toward their NDIS goals

Advocate for client rights, preferences, and autonomy

Contribute to positive outcomes through consistent, person-centred care
