About the role
The Residential Aged Care Liaison plays a vital role within the Social Work and Counselling Department, working closely with multidisciplinary teams across South West Healthcare (SWH). This position supports safe, effective, and patient-centred transitions for individuals aged 65 and over into permanent or respite residential aged care. Acting as a key contact for aged care placements and discharges, the role is both flexible and responsive to the unique needs of each patient and their family. Drawing on a deep understanding of the aged care sector, the Liaison helps plan future care and ensures a smooth, well-coordinated transition that aligns with organisational goals and patient wellbeing.

- Classification: Enrolled Nurse
- Salary Range: $51.11 per hour, including casual loading
What you bring
- An Enrolled Nurse holding a current registration certificate as required by APHRA
- Minimum of 5 years' relevant experience
- Demonstrated understanding of the aged care sector and relevant legislation
- Demonstrated proficient negotiating, interpersonal and interdisciplinary skills
- Demonstrated knowledge of residential aged care placement processes, services, networks or ability to develop skills
- Ability to work independently and problem solve
- Commitment to person and family centered care
- Understanding of the Victorian health system and local organisational systems and an ability to identify and address systemic issues faced by patients and families and act on them appropriately
- Well-developed written and verbal communication and interpersonal skills
- Capacity to work skilfully and creatively in an environment of competing values
