About the role:
We are expanding and looking for an experienced and passionate NDIS Support Coordinator / Psychosocial recovery Coach to join our growing team.
In this role you will work closely with participants and families, to implement and navigate their NDIS plan, build sustainable support networks and achieve meaningful outcomes.
You’ll be the go-to connection between participant, providers, and all stakeholders involved in the participants team.
Key Responsibilities:
· Develop & review individualised support plans in collaboration with participants and their families/ key stakeholders
· Coordinate a diverse range of supports including allied health, housing, employment and community engagement.
· Support participants to understand their & effectively use their NDIS funding, including using budgeting programmes to achieve the budgets for the plan.
· Liaise with service providers, NDIA representatives, and community stakeholders.
· Monitor progress goals, & prepare reports to the NDIS ie: plan implementation and progress reports
· Advocate for participant needs to ensure the best possible outcome
· Maintain accurate and timely documentation in line with NDIS quality standards
What we are looking for:
· Full Time 8.30am -4pm
· Minimum 2 years’ experience in a Support Coordinator/ psychosocial recovery coach role
· Proficient in using computer programmes, such as Microsoft package.
· Strong understanding of the NDIS framework, pricing agreements, and person-centred practice
· Excellent interpersonal, organisational and advocacy skills and time management skills
· Capacity to manage multiple caseloads with autonomy
· Proficiency with client management systems and basic tech skills
· Current drivers licence and access to a reliable fully insured vehicle.
· Current WWC card (Blue Card) and NDIS Screening Card (Yellow Card)
· Current CPR and First Aid certificates
