Role Purpose Summary

The Circle Program is a funded Therapeutic Foster Care service and is a collaborative partnership between OzChild and the Australian Childhood Foundation (ACF). OzChild Home Based Care program holds responsibility for the case management aspect of this program.

The major emphasis of the Circle Program is to provide high quality care based on a thorough understanding of the child. A therapeutic care plan is developed for each child which informs the care team. The care team is comprised of the Circle Senior Case manager, ACF therapeutic specialist, birth family, carer, DHS case manager and other persons as appropriate. Regular care team meetings are held which promote strong communication between key adults and consistency of approach when working with the child. Intensive support is provided to the carer who is viewed as being best placed to provide therapeutic support to the child.

The purpose of the position is to improve outcomes for children and young people by delivering services and program outcomes through the support, supervision, and development of clients in the Foster Care program.

The role develops and implements plans to deliver positive planned outcomes for vulnerable children or young people in foster care. The role works to deliver on the permanency objective to provide safe stable care for clients. The role represents the clients’ voice and ensure the needs of the child are consistently met.

As a Senior Case Manager – Circle Program, the incumbent draws upon their advanced expertise to:

• Provide informal peer support, coaching and mentoring of other team members

• Leadership in the management of complex situations or stakeholder management.

• Contribute actively to the building of the OzChild profile through networks and conferences

• Initiate opportunities to inform improvement in processes and achievement of client and carer outcomes.

Qualifications

Essential

Bachelor’s degree in social work or equivalent that is recognised in Australia

  • Demonstrated success in delivering outcomes in a client focussed program
