ABOUT THE ROLE
We’re looking for an experienced Level 2 Support Coordinator to join our team, delivering high-quality services to NDIS participants. This is a contract position offering truly flexible hours and the ability to work from home. While the role is primarily home-based, it does involve travel to participants' homes and community settings across the Gold Coast and Northern Rivers, NSW
Responsibilities include:
Empowering participants to take the lead in shaping their lives in ways that are meaningful to them.
Coordinating a range of services to support participants in living fulfilling, inclusive lives within their communities.
Building and maintaining strong relationships with key stakeholders- including government agencies, community organisations, clinical providers, and families.
Assisting participants in developing their capacity to build and maintain a strong support network, including services, community connections, and personal relationships.
Helping participants understand their NDIS plans, goals, and objectives while encouraging choice, control, and independence.
Preparing for Plan Reviews and Attending with the Participant, Family, or Representative
ABOUT YOU
To be successful in this role, you will possess:
Relevant Tertiary qualifications in Community Services, Mental Health and Disability/and or Similar
1+ years of experience in a similar role.
Excellent interpersonal and communication skills.
Strong written communication abilities.
Demonstrable problem-solving skills.
The capacity to collaborate effectively.
An understanding or knowledge of the NDIS.
Familiarity with psychosocial disabilities (preferred).
Relevant qualifications in disability services, social work, or related fields (desirable).
Essential Requirements:
Current NDIS Worker Screening Check and Working With Children Check
Current First Aid and CPR Certification
Completion of NDIS Mandatory Training
A valid driver’s license and reliable, insured vehicle.
