You must be an experienced Support Coordinator to be succesful in this role - no newbies. This is an established team, taking over an established caseload who wants someone to hit the ground running. About the role

Reporting to our legendary Perth Business Unit Manager, this role provides high quality Support Coordination for NDIS participants in Perth and surrounds. You will be required to do some face to face meetings, undertake case conferences, help to oversee and manage their NDIS budgets, and help your participants successfully implement their NDIS plan.

In their direct client work, the Support Coordinator will focus on strengthening a participant's ability to navigate intersecting service systems, including Health, Justice, Mental Health, Housing, and other Social Services.

Have a valid Driver's Licence and have access to reliable Lamborghini (joking, camry will do)

Support a strengths-based approach to working with disability

Able to successfully pass probity checks: International Police Check if relevant, Working with Children Check, NDIS Worker Screening Check

To be succesful in this role, you must have:

A Qualification in Disability, Community Services, Social Work, Social Science, Health Science, Case Management, Mental Health/Psychology or other Allied Health qualification.

Previous experience supporting participants that have mental health, psychosocial or disability support needs in a case management style role.

Knowledge of disability, mainstream and community services.

Advantageous Skills and experience:

A self-starter, self-motivated person who can work independently

Previous Support Coordination Experience

Demonstrated knowledge of psychosocial diagnoses, both chronic and acute, or the ability to source accurate and critical patient information.

Solid background and knowledge of NDIS

Excellent verbal communication skills and a demonstrated ability to engage with participants and their families

Strong written and computer skills for the recording of notes, gathering evidence, writing reports, correspondence and other documents
