Health Outreach Worker
Part -Time – 22.8 Hours Per Week | Flexible Working Days
Fixed Term, 24-Month Contract

About the Role
The Health Outreach Worker helps Aboriginal and Torres Strait Islander people access health services while ensuring that those services are culturally appropriate. This is a non-clinical support role, with responsibilities that include:
Community liaison
Administration and program support
Providing practical assistance to clients
Offering feedback about access issues and service gaps
Transporting patients to dialysis and specialist services
About You
You will bring the following knowledge and skills:
A commitment to maintaining client confidentiality
Understanding of and dedication to Aboriginal and Torres Strait Islander culture
Initiative and the ability to work independently
Strong coordination skills to collaborate across teams and programs
Empathy and understanding of the needs of people with mental health concerns, chronic illness, disability, or dementia
Willingness to provide transport support, including outside regular business hours (early mornings or evenings as needed)
What We Offer:
Competitive pay and conditions based on the ACCHS Award – Community Health Worker Grade 1 (based on qualifications and experience).
Salary packaging of $15,900 available for Part- or Full-time employees.
