About the opportunity
Are you a passionate and community-minded individual who thrives on connecting with others and making a real difference? Do you have a deep understanding of your local community and a knack for helping people navigate support services? If so, we're looking for Carer Hub Navigator -who will be the welcoming face and guiding hand for carers in our new Carer Hubs.
This is an exciting new role designed to integrate community development and community engagement approaches to raise awareness and enhance accessibility to Carer Gateway services through in our Carer Gateway Hub.
The Carer Services Navigator role is based in the Townsville region, providing consistent presence and reliable support to carers however adopts a 'no wrong door' approach. Working in collaboration with a team, you will actively build strong, sustainable relationships with community organisations, health services, schools, aged care facilities and local government agencies to further strengthen and embed carer support locally.
The role has four core functions:
To create seamless connections between Carers and vital community resources, cultivating meaningful relationships that support wellbeing and independence. Supporting Carers to thrive, focusing on holistic well-being, going beyond basic needs to help carers achieve their full potential. The service is Carer focused, 'how can we help' and a 'no wrong door approach'. Oversee the Townsville Carer Hub, facilitating social interaction and promoting wellbeing, connection and a sense of belonging.
To be successful in this role, you will need:
- Experience working with family carers in mental health, alcohol and other drugs, community support services
- Diploma or above in a support discipline and/or Equivalent Community sector experience
- Current valid Driver's Licence
- Strong technology skillsl,
- Satisfactory National Police Records Check (within 12 months)
- WWCC / QLD Blue Card
- Child Safety and Personal History Screening (LCS2)
- NDIS Workers Screening check / QLD Yellow Card
- Appropriate IT Skills
- Right to Work within Australia
- NDIS Worker Orientation Module Certificate
