About the role
* Full-time/Part-time/Casual/Contract based Community Occupational Therapists:
Melbourne ( North & western suburbs)
Active Allied Health Australia Pty Ltd' is seeking a motivated and client-centred Occupational Therapist to join our community team, supporting older Australians living at home with funded Home Care Packages (HCP). In this role, you will help clients maintain independence, safety, and quality of life through functional assessments, assistive technology, and environmental interventions.
A hybrid role to suit your work life balance with multiple positions available.
Key Duties & Responsibilities
Conduct comprehensive functional and home safety assessments for HCP clients.
Recommend and prescribe assistive equipment, aids, and minor/major home modifications in line with aged care funding guidelines.
Develop and implement individualised intervention plans to support clients’ goals for independence, mobility, and self-care.
Collaborate with case managers, support coordinators, support workers, GPs, and other allied health professionals.
Provide clear and evidence-based clinical reports and funding justifications for services and equipment under the HCP budget.
Deliver education and training to clients, carers, and support staff regarding safe use of equipment and activity strategies.
Promote reablement, wellness, and preventative health approaches in all interventions.
Maintain accurate and timely documentation in compliance with Aged Care Quality Standards and organisational policies.
Participate in regular clinical supervision and professional development activities.
What we're looking for
Degree or postgraduate qualification in Occupational Therapy
Current registration with the Australian Health Practitioner Regulation Agency (AHPRA)
Minimum one years' experience in a community-based occupational therapy role
Excellent communication and interpersonal skills, with the ability to build rapport with clients and their families
Strong problem-solving and critical thinking skills to address complex client needs
Familiarity with assistive technology, home modifications and other occupational therapy interventions
Commitment to evidence-based practice and a focus on client-centred care for HCP clients.
