Your Opportunity
Looking to make a real impact with young people? Join FLO as a Case Manager and help high school students re-engage with learning and life! You'll work one-on-one with youth aged 15-21 (or 25 for young parents), creating personalised learning plans and providing the support they need to thrive. Your key responsibilities will be to:
- Respond to referrals of clients to the service from internal and external support services and conduct over-the-phone and formal face-to-face assessments of suitability for support
- Register and induct clients into the service including the property, facilities, financials and regulations
- Provide ongoing case management sessions (formal and informal) with clients and review progression against case plans and provide informal counselling as required
- Assist clients in the process of transition out of the service into independence or other services
- Development of an agreed individual learning plan (ILP) in conjunction with (if needed) an agreed case plan including goals and outcomes
- Develop strong internal and external relationships with clients, staff, and key stakeholders to contribute to the effective functioning of the service and improved outcomes
Requirements for Success
- Certificate IV or Diploma in Youth Work or Youth Justice, Bachelor of Psychology or Bachelor of Social Work
- Previous experience working in a case management approach within a education or employment focused environment
- Previous training in providing a Child Safe Environment, Reporting Abuse & Neglect and Education in Care
- Current Working with Children Check
- Current Drivers Licence
- First Aid Certificate
Before starting work with us, you will need to undertake a national police history check, qualifications, and referee checks, and have a clearance to work with children/vulnerable people.
