The Position

The Shire of Manjimup is seeking a suitably qualified and experienced individual to join the Shire’s Home and Community Care team. This role is responsible for:

Assessing the needs of individuals and their service requirements whilst promoting physical and emotional wellbeing.

Developing support plans for individuals residing in the Respite House and report required changes to the Clinical Care Coordinator or Manager Home & Community Care.

Assisting with clinical aspects of HCP consumers support when required.

Ensuring Support Workers and EN’s under your supervision are appropriately skilled and understand the needs of the consumer.

Administering Schedule 8 medications and provide specialised nursing care as per consumers clinical support plan in the Respite House and in home care.

Providing high quality supervision of the Respite House, ensuring the respite service runs effectively and efficiently on a day-to-day basis.

Available for flexible hours during the day and weekends, including overnight on call shifts.

Liaising with medical professionals, allied health professionals, co-workers and other personal in order to provide the best outcomes for individuals.

Assisting with clinical training to Respite House and Community workers.

Selection Criteria

To be successful in this role you will need:

Current AHPRA Registration as a Registered Nurse.

3+ years nursing experience within the Aged & Disability care sector.

Demonstrated advanced clinical experience and expertise working with consumers in the community.

Understanding of care assessment in the Aged Care sector.

Knowledge and experience with supporting people living with dementia or a disability.

Excellent computer literacy and communication skills both verbally and written.

Current 'C' class driver's licence and a registered reliable vehicle with full comprehensive insurance is required.

Knowledge of Aged & Disability legislation and standards (desirable)

Knowledge of reporting requirements as prescribed in the contracts with the relevant funding bodies (desirable).
