As an Aged Care Connector (ACC), you will play a vital role in empowering Aboriginal and Torres Strait Islander Elders by connecting them with the aged care services they deserve. Your efforts will ensure that our Elders receive the support and care they need to live with dignity and respect.

In this role, you will be the bridge between our community and essential aged care services. Through active outreach and personalised support, you will help clients navigate the complexities of aged care, advocate for their needs, and facilitate access to vital resources. Your work will not only enhance the quality of life for our Elders but also strengthen the bonds within our community.

Connect with local Elders, older Aboriginal and Torres Strait Islander people, and their families to raise awareness of their aged care entitlements and ensure effective access to appropriate aged care services.

Key Responsibilities:

Promote aged care services and engage with potential clients through community outreach.

Support clients in identifying their needs and accessing appropriate care and services.

Assist with advocacy and support during My Aged Care registration and review processes.

Facilitate client access to appointments and collaborate with the aged care team.

Ensure compliance with safety practices and participate in quality improvement processes.
