About the opportunity
IFYS has an exciting opportunity for an Outreach Case Manager to join the Accommodation Services team within the Gympie region.
As the Outreach Case Manager, you will provide assertive outreach support to rough sleepers and individuals experiencing housing crisis, within a holistic and collaborative multi-agency model of service.
Our ideal candidate will possess a strong client-centred practice framework, have sound knowledge of local services and resources, and experience conducting intake assessments and delivering case management.

- Salary base $88,099.44 per annum + super – IFYS is committed to gender pay equity

Key responsibilities
- Provide place based assertive outreach to individuals and families experiencing homelessness or those at risk of homelessness within the Gympie LGA.
- Assist clients through the provision of case management to access and engage with services to address their individual needs.
- Work with other providers within a multi-agency team, to provide clients a holistic and collaborative model of service.
- Provide emotional, social and welfare support to individuals experiencing housing crisis in a culturally safe, therapeutic and trauma informed manner.
- Assist individuals with applications for housing and their transition to longer-term accommodation options.
- Complete administrative tasks, including maintaining service user records, meeting minutes, the completion of daily logs, and the provision of accurate and timely reports as required.
Requirements for the role
- Demonstrated professional experience delivering case management to individuals experiencing homelessness
- Ability to be able to operate independently
- A relevant tertiary qualification, Diploma level or higher
- Comprehensive knowledge and understanding of Homelessness Sector within the greater Sunshine Coast and Gympie areas
- Excellent interpersonal and communication skills, with the ability to build and maintain effective relationships with all stakeholders
- Sound judgment, problem-solving skills, and the ability to handle crisis situations calmly and effectively
- Exceptional time management and prioritisation skills
- Demonstrated experience in maintaining program compliance
- A continuous learning mindset
Must hold or be willing to obtain if successful
- Valid Queensland driver's license with no restrictions
- QLD Working with Children Check (Paid Blue Card)
- Licensed Care Suitability Check (LCS2)
- National Crime Check (IFYS will complete a new check through our systems)
- Ability to provide two recent line manager references including your current employer
