Join Mid Murray Support Service (MMSS) as a Care Plan Advisor, supporting older Australians to live safely and independently at home.

About the Role:

· Permanent Part Time position with immediate start (25 Hours per week)

· Coordinate and review in-home aged care & disability services

· Liaise with clients, providers, and internal teams

· Ensure care plans align with client goals and budgets

· Advocate for clients through onboarding, assessments, and reviews

What You’ll Need:

· Cert III in Individual Support (Ageing & Disability) or equivalent (required)

· Aged Care or NDIS coordination experience

· First Aid + CPR, valid Driver’s Licence, own insured vehicle

· Strong admin, tech, and communication skills
