Are you a Registered Nurse looking to make a real difference in the lives of young people? If you have a passion for holistic support and primary health care, we have the perfect opportunity for you at The Junction Youth Health Service.

What's the role about?

As the Practice Nurse, you'll be part of our amazing multidisciplinary team, providing essential health care to young people aged 12 to 25. Your focus will be on those who are homeless or at risk, ensuring they receive the best possible care.

What you'll be doing:

Conduct health assessments, treatments and provide education and undertake cross-sector service co-ordination.

Work closely with practice GP's, external medical and mental health services, hospital services, community services and allied health services.

Be a voice for young people, ensuring they receive the care and support they need.

Keep thorough records of all care provided, to meet medico legal standards and data collection requirements of funders.

Participate in outreach clinical activities via our Youth Health van, delivering services to key community service locations and specialist schools.

What we're looking for:

Registered Nurse with ongoing participation in continuing professional development.

Minimum of 5 years' post registration experience in nursing, with knowledge of the needs of vulnerable young people and their families.

Strong organisational, planning, and communication skills, with the ability to work collaboratively within a multidisciplinary team.

First Aid Certificate, CPR, Driver's Licence, Working With Vulnerable People Check
