About the Role:

The Community Care Program Social Work Manager is part of the Community Care Program within the Division of Rehabilitation Aged and Community Services (RACS). In coordination with the broader Community Care Program leadership team, this position is responsible for managing operational requirements, coordinating and providing clinical leadership, supervision, mentoring and carrying a clinical caseload. The role supports clinicians across a range of community health centres providing social work to community-based clients through outpatient services at community health centres, telehealth and some home visits including clients accessing the Community Assistance and Temporary Support (CATS) Program and Rapid Assessment of the Deteriorating Aged at Risk (RADAR). The role requires clinical knowledge and skills to support adult clients presenting with a broad range of psychosocial needs impacted upon by health issues. Client's healthcare needs may vary, but will predominately include those with chronic conditions, complex medical and psychosocial conditions, and frail aged conditions.

  • Current registration issued under the Working with Vulnerable People (Background Checking) Act 2011 is required.
  • Comply with CHS credentialing and scope of clinical practice requirements for allied health professionals.
