The Community of Cypriots of the Northern Suburbs of Melbourne Inc. is seeking a well-established Home Care Coordinator who will be responsible for the setup of a newly commencing service within our organisation.

Experience of 5-7 years in delivering services in a home care environment will be required.

The Home Care Coordinator is responsible for coordinating and monitoring the delivery of services under the new Support at Home Program, ensuring high-quality, person-centred care that promotes individuals’ independence and wellbeing. Working in partnership with individuals, families, and care providers, the coordinator ensures that services are aligned with individuals’ support plans, delivered within budget, and compliant with government funding guidelines and program changes.

The role will involve liaising with appropriate service suppliers such as Occupational Therapist, Physiotherapists, garden maintenance, carers, nurses etc.., to establish service agreements for the commencing service.

This role will also involve scheduling care and services, monitoring individuals’ outcomes, maintaining accurate documentation, and supporting the integration of clinical, allied health, and non-clinical supports as required. The Home Care Coordinator will play a key role in adapting to the evolving Support at Home requirements, contributing to quality assurance processes, and ensuring a seamless transition for individuals from existing programs (e.g., HCP, CHSP) to the new model of care.

Strong communication, system navigation, and administrative skills are essential to support clients effectively and ensure compliance with new regulatory and reporting expectations.

Relevant Qualifications in Community Services, Aged Care and Management are essential.
