We're Looking For:

Experienced Case Managers with strong knowledge of the community or aged care sector

Prior experience in coordinating home care services (HCP or private) preferred

Excellent interpersonal and communication skills

Highly organised, detail-oriented, and proactive problem solver

Compassionate and client-focused with a genuine passion for care

A current driver's licence and willingness to travel locally for assessments

Your Key Responsibilities Will Encompass:

Conducting in-home or facility-based client assessments

Developing, implementing, and reviewing personalised care plans

Coordinating service delivery and delegating tasks to care staff

Managing client budgets (especially for government-funded programs)

Liaising with families, health professionals, and service providers

Acting as a trusted point of contact and advocate for client wellbeing

About You:

To be successful in this role, you will ideally have:

Qualifications in care, community services, or allied health (preferred but not mandatory)

Previous experience in case management or community-based care

A proactive and client-focused approach

Strong attention to detail and time management skills

Police Check, Working with Children Check, full working rights in Australia

Access to a reliable vehicle
