About the role
We are seeking an empathetic, self-motivated Mental Health Practitioner for a fixed-term, full-time opportunity until June 2026, delivering outreach support across the Melbourne metropolitan area for people receiving TAC/WorkSafe funding. In this role you will provide personalised, recovery-oriented support to people living with mental ill-health in the community. This includes one-on-one and group support, case management, coordination of services, assistance accessing services, education and vocational guidance, and recreational activities.
A fleet vehicle will be provided for travel associated with the role
About the Service
The Complex Care Fee For Service (FFS) program offers a range of specialist interventions and approaches to people with psychosocial disability and complex needs who have TAC and WorkCover support packages with the focus on maximising recovery outcomes in a fee for service model
Key responsibilities
- Deliver high quality outreach services, case managed responses and facilitate positive educational and social experiences.
- Coordinate services, and provide mentoring aligned with the goals and aspirations outlined in TAC/WorkSafe recovery plans.
- Support clients with strengths based, recovery-orientated approaches to develop daily living skills, capacity and resilience.
- Provide information and mentoring to clients to establish connections with community, families and carers, create networks and access to relevant services.
- Collaborate with a range of services, families and the community to ensure a co-ordinated, integrated response to client’s recovery goals.
What you’ll bring
- Tertiary qualifications (minimum Certificate IV) in Mental Health, Peer Work, Social Work, or other health-related field.
- Experience and expertise in working directly with people with mental health issues, complex needs and with their families and carers in an outreach roll.
- Strong understanding of trauma-informed and recovery-oriented practice.
- Ability to work both autonomously and collaboratively showing initiative and flexibility.
