About the role:
Multicultural Australia is currently offering an opportunity for a Care Finder Coordinator to work as part of our Client Services team. The aim of the Care Finder Program is to deliver services to older adults from Culturally and Linguistically Diverse Backgrounds (CALD) within the care finder target population to provide specialist and intensive assessment to support clients to establish their goals, develop appropriate strategies and actions, develop a support plan based on their needs, and match and refer them to appropriate services of their choice, to promote their ongoing wellness and achieve their goals.
This is a Part-Time role, based in Brisbane and Redlands, ideal for someone passionate about community engagement and supporting people from diverse backgrounds.
What You’ll Be Doing:
Delivery of high-quality care finder support through information, support, representation and assertive outreach;
Complete comprehensive psychosocial needs assessments with individual clients and families. Development of case management plans which focus on addressing individual aged care needs and goals;
Prepare for assessment by contacting the client/carer/representative to arrange appointment within the required timeframe and complete the pre-assessment Home Safety checklist;
Facilitating holistic support that enhances clients’ emotional, psychological, and physical wellbeing, using trauma-informed and culturally sensitive approaches;
Contributing to program development, evaluation, and continuous improvement;
Support older adults from a Culturally and Linguistically Diverse (CALD) backgrounds to interact with My Aged Care portal to improve awareness and build capacity to navigate the system so they can be screened for eligibility and referrals for aged care services;
Provide specialist and intensive assistance to help individuals understand and access
aged care services and connect with other relevant supports in the community;
Providing assertive outreach and direct client support across Brisbane, Redlands, and occasionally Scenic Rim and Bay Islands.
Qualifications and Skills Required:
You will be comfortable working in a fast-paced environment and have effective communication and interpersonal skills. As well as having a confident and positive personality, you will possess:
Possession of Bachelor level tertiary qualification in the field of Social Work; Psychology, Applied Social Sciences, Counselling or other relevant allied health disciplines;
Relevant qualifications (e.g. social work, human services, aged care, community
services or health) and/or relevant experience;
Demonstrated knowledge and experience of casework frameworks in a multicultural context;
Able to prioritise and respond to competing demands in an organized and timely manner;
Strong verbal and written English language communication and cross-cultural communication skills and an excellent understanding and knowledge of issues in relation to support of Humanitarian entrants and migrants, as well as of current practice and policy issues;
Successful candidate will be required to have a valid driver’s licence, police check and apply for a Working with Children Blue Card.
Additional Desirable Skills:
Experience working in the not-for-profit sector.
General administrative experience to including the use of computer-based calendar and information management systems.
