This medium-sized Aged Care Facility, passionate about providing experienced, flexible and innovative care solutions to ensure residents are confident and their current and future care needs are met. An exciting opportunity has become available for an experienced and passionate IPC Clinical Care Coordinator to join their team in The Southern Highlands on a 7 month contract with the with potential to be transitioned to a permanent role.
Job Role and Summary:
Reporting to and working alongside the Care Manager, you will play an integral part of delivering high-quality care. To be successful in this role you will have utmost respect, empathy and a passion for residents and their families. Some of the responsibilities will include:
- Providing quality resident-focused and clinical care
- Supporting the Care Manager to ensure high quality personal and clinical care needs are met and align with legislation requirements and best practice.
- Maintain accountability and responsibility for the clinical management of health and care outcomes of the residents
- Undertake assessments, audits, ongoing education, treatments and documentation to the highest standards
- Provide clinical leadership in the coordination of nursing and care services
- Take an active role in getting to know the residence and families
- Working closely with the Care Manager and leadership team to ensure consistent compliance with all Aged Care standards and Quality Care policies and procedures
Skills & Qualifications:
- AHPRA Registered Nurse with current registration status
- Minimum 3 years’ experience in the same or similar role
- IPC Accredited
- An excellent understanding of the Aged Care Act with knowledge and experience of the Australian National Aged Care Classification (AN-ACC) Funding
- Sound understanding of the Aged Care Quality Standards
- Advanced clinical experience
- Strong leadership skills with the ability to maintain collaborative relationships
- Ability to foster a safe workplace and drive continuous quality improvement initiatives to develop sustainable practice
- Good computer skills and experience with Autumn Care/Le Care
- Excellent communication and interpersonal skills
