The Role
You will be providing Physiotherapy services to clients through In-Home Care funding (HCP, CHSP and STRC packages). Some regular duties include:
Providing 1 on 1 evidenced based in-home Physiotherapy services such as exercises, rehab and pain management.
Assessment of mobility and function for the provision of mobility and functional/exercise equipment.
Implementing outcome measures - allowing for goal setting and accountability for clinical outcomes.
As part of a Multi-D team, collaborate with other PTs & OTs (and other allied health), nurses, support workers, doctors and coordinators to ensure clients achieve their goals.
Liaise with families, specialist services and other stakeholders as appropriate.
Skills and Qualifications
Bachelor or Master’s Degree in Physiotherapy and current registration with AHPRA
Valid Driver's license and own reliable and insured vehicle
Valid NPC, or willingness to obtain
Current Flu vaccine and COVID vaccine certificate or willingness to obtain
Additionally, the successful candidate will have the following:
Initiative and strong organisational and time management skills
Strong communication skills
Ability to build and maintain meaningful relationships with clients and other stakeholders
Proficient computer skills including the ability to use Practice Management Systems desirable (Nookal or similar)
Some knowledge of the requirements of the Aged Care Standards
