We are currently recruiting for a Registered Nurse to work for a fantastic not for profit in the Inner City / East Melbourne area. This organisation supports older people who live in the local community - including those with Home Care Packages. The focus for this role would be to carry out assessments, provide case management services and provide clinical advice. The role is initially for a 3 month period with the strong chance of extension / a permanent role. Whilst this is a Registered Nurse position - the nursing component would be more to use your nursing experience to inform care planning etc, as opposed to the practical elements of nursing.

About the role

• We are looking for someone with previous community aged care experience - ideally someone with Case Management experience - specifically someone who has experience working with older people who have home care packages
• Full Time position - initially for 3 months with a strong chance of extension / a permanent role
• Hybrid / remote work arrangements available - would be required to be located in Melbourne to enable you to see clients and attend the office 2 days per week
• Training and support provided
• Based out of the Inner City / East Melbourne area
• Hourly rate from $50 - 60 per hour, plus super and KM / travel reimbursement

This organisation have been supporting people in the community for many years and have a range of services which they offer, including a big focus on supporting older people. They are a larger not for profit organisation and invest heavily in training and the development of their staff.

Duties:

• Conduct assessments and provide case management services for older people who live in the community
• Provide clinical guidance and advice
• Oversee budgets and care plans
• Monitor, evaluate and review care plans as required by client care needs
• Liaise with other services providers, including allied health professionals, care staff and other support services
• Network with other service providers
• Ensure all services are addressing client needs according to care plan

Requirements:

• We are looking with previous experience working with older people, in particular someone with Case Management experience
• It is an essential requirement to be an AHPRA Registered Nurse
• We require someone located in Melbourne, with full work rights
