Country Connect Foundation is a First Nations-led not-for-profit organisation committed to delivering culturally grounded crisis accommodation and homelessness support in remote Aboriginal communities across the Northern Territory. Our mission is to walk alongside individuals experiencing housing insecurity by providing safe, respectful, and culturally responsive support, while prioritising connection to country, culture, and community.

The Role
We are seeking a compassionate reliable Case Managers to join our Crisis Support Accommodation team. This role supports individuals and families who are experiencing, or at risk of, homelessness - providing a safe, calm, and culturally respectful environment during a time of transition.

Working as part of a committed team, you’ll deliver trauma-informed, strengths-based care that responds to each person’s unique needs. You’ll assist with daily routines, emotional support, and access to services, while maintaining a safe and welcoming space.

Key Responsibilities:

Overseeing case management practices and ensuring strong housing pathways

Provide day-to-day support to individuals and families staying in crisis accommodatio

Maintain a safe, clean, and culturally safe living environment

Offer practical assistance with meals, personal care, and daily tasks as needed

Follow each resident’s support plan and contribute to their ongoing goals

Work alongside a multidisciplinary team, including case managers, to support long-term housing pathways and wellbeing outcomes

Support residents to access health care, housing services, and other essential supports

Build respectful, trusting relationships grounded in cultural humility and trauma-informed practice

Accurately complete records, incident reports, and shift handovers

Respond appropriately to crisis situations and escalate concerns when required

Building relationships with Aboriginal organisations, housing services, and community partners

Embedding local cultural leadership and ensuring services are safe, inclusive and accountable

About You
Essential Criteria:

Understanding of trauma-informed care and the importance of cultural safety

Comfortable supporting people with complex needs or challenging circumstances

Reliable, calm, and respectful in your approach

Excellent communication and interpersonal skills.

Ability to work independently and as part of a cross-cultural team.

Ability to collaborate with stakeholders to deliver integrated support.

Valid NT Driver’s License.

Working with Children Check (Ochre Card).

NDIS Worker Screening Check (or willingness to obtain).

First Aid & CPR Certificate (or willingness to obtain).

Desirable Criteria:

Experience working in housing, homelessness, or community services desirable

Certificate III or IV in Community Services, Mental Health, AOD, or a related field

Understanding of First Nations cultures and communities is highly regarded

4WD driving skills and confidence in off-road travel.
