About the role

We are seeking an empathetic, self-motivated Community Mental Health Practitioner for a fixed-term, full-time opportunity until 31 October 2025, delivering outreach support across Port Lincoln and the Eyre and Far North region. In this role you will provide personalised, recovery-oriented support to people living with mental ill-health in the community. In this role, you will support people living with mental ill-health in the community to achieve their recovery goals, build confidence, strengthen social connections, and live independently. The role involves outreach support combined with administration duties that can be performed from home. A fleet car is provided for travel.

About the Service

Mind’s Outreach Psychosocial Services South Australia (OPSSA) provide person-centred, psychosocial support, including goal setting, transition planning, and assistance with application and transition to NDIS services. The services comprise a two-tiered approach to support:

  • Tier 1: Intensive Crisis Support

Provide flexible, intense support and case management for a limited period (6 weeks) to reduce immediate distress, enhance personal safety, and connect clients to long-term community services.

  • Tier 2: Non-acute Psychosocial Support

Offer moderate support over a four-month period (extendable to 12 months) to help clients achieve recovery goals, build daily living skills, maintain housing, and foster social connections. Assist clients with NDIS eligibility, application, and transition.

Key responsibilities

  • Deliver high quality outreach services, case managed responses and facilitate positive educational and social experiences.
  • Plan and deliver group programs that aid clients to build their skills and focus on recovery.
  • Support clients with strengths based, recovery-orientated approaches to develop daily living skills, capacity and resilience.
  • Provide information and mentoring to clients to establish connections with community, families and carers, create networks and access to relevant services.
  • Collaborate with a range of services, families and the community to ensure a co-ordinated, integrated response to client’s recovery goals.

What you’ll bring

  • Tertiary qualifications (minimum Certificate IV) in Mental Health, Psychology, Social Work, Occupational Therapy or other health-related field.
  • Expertise in working with people with mental health issues, complex needs and interacting with families and carers.
  • Experience applying therapeutic approaches such as trauma-informed care, cognitive behavioural therapy, motivational interviewing, harm reduction, and family-inclusive practice.
  • Demonstrated understanding of the Community Services sector and ability to build strong relationships with service providers, clinical teams and key stakeholders.
