$45.66 to $52.49 per hour + salary packaging | Occupational Therapist - Grade 2
Permanent Full Time or Part Time | 1.20 EFT available | Support at Home Program (Linkages Ballarat)
About the role
Grampians Health is seeking passionate and skilled Occupational Therapists to join our team the Home Care Packages program (Linkages Ballarat). You will join a large Occupational Therapy team at Grampians Health with programs across Acute, Subacute, Community, and other specialty areas.
In the Support at Home program, you'll be part of a collaborative, client-focused Home Care Packages team that values innovation, compassion, and holistic care. We offer structured supervision, supportive leadership, and a focus on professional development.
Be part of our Linkages Ballarat team, providing Occupational Therapy services to clients living at home under the Home Care Packages program. You will work closely with clients, families and care teams to deliver holistic, community based Occupational Therapy care.
- Provide home based OT assessment and intervention
- Support older adults and people with disabilities to achieve their goals
- Contribute to care planning and program development
- Collaborate with a multidisciplinary Home Care Packages team
- Participate in supervision, mentoring and student support
What we're looking for
- AHPRA registered (or eligible) Occupational Therapist
- Bachelor's degree in Occupational Therapy (or equivalent)
- Valid Victorian Driver's Licence (essential)
- Strong interpersonal and communication skills
- Self-motivated, client-centred and organised
