Accessible Care is a trusted NDIS-registered provider offering high-quality, person-centred support across Victoria and South Australia. We are currently seeking compassionate and experienced Support Workers to join our growing team in Reservoir, VIC.

This role involves providing in-home support to clients requiring palliative care, including those with Motor Neuron Disease (MND). You will work closely with families, clinical staff, and our internal allied health team to ensure clients receive safe, respectful, and dignified care.

Key Responsibilities:

Provide personal care and support with daily living activities

Deliver palliative care with empathy and professionalism

Support clients with chronic conditions, including MND

Administer medications as per care plan (must hold valid Medication Administration Certificate)

Maintain accurate and timely documentation

Collaborate with allied health professionals and family members

Promote client independence and quality of life

Medication Administration Certificate (HLTHPS006 or equivalent)

Manual Handling Support

Personal Care Support

NDIS Worker Screening Check (NDIS Clearance)

Essential Requirements:

Minimum 6 months experience in disability or aged care

Proven experience in palliative care and MND Management

Current First Aid & CPR Certificate

Valid Driver’s Licence and access to a reliable vehicle

Strong communication and interpersonal skills

Empathetic, patient, and reliable approach to care
