Hours: 38 hours per week Monday to Friday
Salary Package: $88,099- $100,783 plus superannuation, annual leave and personal leave entitlements.

Support Coordination / Psychosocial Recovery Coaching (Social Worker/New Grad)

Start Date: Immediate Start
Job Type: Permanent, Part Time
Hours: 20 hours per week Monday to Friday
Salary Package (Part Time): $36,400 plus superannuation, annual leave and personal leave entitlements.

We provide services for anyone wanting professional counselling, support work and support coordination for their unique needs and circumstances. We provide an outreach mobile service as well as in clinic therapy, working in Brisbane and its surrounding areas. We also provide telehealth counselling services throughout Australia. The Happy Peeps office is located in Brisbane's Western Suburbs and you will be required to work in office and community, as required, with a company vehicle provided.

Social Work Therapist / Social Work Behaviour Practitioner

Position Description

Provide counselling and therapy services in practice for adults and children.

Provide evidence based therapies and strategies to individuals from the community.

Behaviour and Functional Assessments and report writing.

Have excellent time management skills and be able to work autonomously.

Have exceptional communication, interpersonal and social skills and work from a client centred approach.

Have a friendly, positive attitude and approach and be willing to participate within a caring team environment.

Be competent with computers and Microsoft suite.

Responsible for case noting and reporting.

Requirements

Bachelor/Master of Social Work (Accredited Mental Health Social Worker advantageous)

Membership with A.A.S.W

Professional Indemnity Insurance

Hold a valid Blue card and Yellow card

Hold a Queensland Drivers Open Licence

Hold a current National Police check

Completed NDIS Worker Orientation Module

Completed Supporting People to Stay Infection Free module

Completed First Aid & CPR Training

Provide a Covid-19 digital certificate

Committed to continuing professional development and awareness of current and emerging evidence-based practices

Support Coordination / Psychosocial Recovery Coaching

Position Description:

Provide Support Coordination and Psychosocial Recovery Coaching to NDIS participants.

Understand and implement NDIS plans.

Encourage capacity building to manage supports effectively.

Connect participants with providers, managing schedules, and ensuring communication between all parties

Liase with stakeholders, community services, government agencies and other service providers.

Demonstrate excellent time management skills and be able to work autonomously.

Possess the initiative to pick up the phone and talk to prticipants

Be able to meet deadlines to ensure participants are receiving our services in a timely manner.

Prepare and maintain up to date and accurate case notes and reports.

Be a key point of contact and communication for the participants

Have an understanding of organisations in the region that provide services to NDIS participants.

Manage consent forms, ensuring follow ups on all tasks.

Understand the frequent NDIS changes, rules and regulations. Stay up to date with any changes.

Requirements:

Bachelor/Master of Social Work (Accredited Mental Health Social Worker advantageous)
