Position Description:
Support Your Way is seeking motivated support workers to join our growing personal care team for persons with disabilities within the Perth Metro - North Area.

The Role
We're seeking compassionate and enthusiastic Support Workers to join our growing WA team. In this role, you'll provide tailored support to NDIS participants, assisting them with daily activities, community engagement, and achieving their personal goals.
Current Opportunities
We are currently looking to fill two roles for young male participants with neuromuscular conditions:
1️⃣ Madeley Participant
Day & Time: Tuesdays, 8:00am – 5:00pm
Support Required: Assistance with daily living and community access
Preference: Female support worker preferred
About the Client: A young, sociable man who values consistency, respect, and independence.
2️⃣ Butler Participant
Hours: Minimum 20 hours per week, with flexible shifts available Monday to Sunday in 3–5 hour blocks
Support Required: Attending appointments, social outings, and general companionship
About the Client: A young man with a passion for getting out and about, looking for someone to connect with and support him in living life to the fullest.
🚐 Bonus: Experience driving a wheelchair accessible van is highly regarded for both roles.
Key Responsibilities
Assist participants with daily living tasks, including personal care and household duties
Support clients in community participation and social activities
Collaborate with clients to develop and implement individualized support plans
Maintain accurate and timely documentation of services provided

Requirements
To be considered for this role, you must have or be willing to obtain:
National Police Clearance
CPR and/or First Aid Certificate
Manual Handling Training
NDIS Workers Orientation Module Certificate
NDIS Worker Screening Clearance
Working with Children's Clearance
100 points of ID
COVID-19 Vaccination Certificate
