Aligned with CALHN's strategic vision for delivering world-class healthcare, Hampstead Centre is launching a new program, “CO-ACT” (Care of the Older Person and Community Transition), focused on providing exceptional care for older adults transitioning from acute to ongoing care. About the Role

We are seeking a dedicated Social Worker to join the Care of the Older Person and Community Transition (CO-ACT) service at Hampstead Rehabilitation Centre. In this role, you will provide assessment and targeted interventions to ensure that consumer needs are considered at the point of admission. You will also support the delivery of placement and community brokerage services in collaboration with the broader multidisciplinary team, ensuring a coordinated and responsive approach to patient care.

This position requires a high level of adaptability, prioritisation, and timely decision-making to manage daily service demands. You will play a key role in supporting smooth transitions of care from admission to discharge, including brokerage and placement support. Flexibility to deliver discipline-specific interventions during periods of staff leave or increased service demand may also be required. The role contributes to effective patient-centred care and may involve participation in quality improvement and research initiatives.

About You

You are a qualified and experienced Social Worker with a strong foundation in delivering patient-centred care across acute, sub-acute, and community settings. Your advanced clinical reasoning, organisational ability, and adaptability enable you to respond effectively to complex patient needs within a fast-paced, multidisciplinary environment.

Hold an appropriate degree or equivalent qualification, with eligibility for full membership of the Australian Association of Social Workers.

Demonstrated experience managing complex psychosocial presentations and delivering high-quality interventions for patients with diverse health conditions.

Proven ability to prioritise, problem-solve, and adapt to shifting service demands, with a commitment to professional development and evidence-based practice.

Strong interpersonal, clinical supervision, and communication skills, with a sound understanding of health service systems, documentation standards, and WHS responsibilities.
