We are seeking a qualified and experienced Physiotherapist to partner with our clients, providing tailored physiotherapy and rehabilitation programs that promote healthy ageing, physical function, and community engagement. You will manage a caseload of community-based clients, delivering safe, effective, and individualised care that reflects their needs, goals, and preferences.

Key Responsibilities:
Therapeutic Planning and Clinical Delivery:
Conduct comprehensive assessments to identify clients' mobility, physical health, and lifestyle goals.
Develop individualised treatment plans and deliver evidence-based physiotherapy interventions to enhance mobility, independence, and overall wellbeing.
Prescribe mobility aids and recommend home modifications to support safe daily living.
Implement strength and balance programs, including falls prevention and home exercise plans.
Provide specialised care for clients with respiratory or cardiac conditions, including cardiopulmonary support.
Regularly review and evaluate therapy plans to ensure client goals are achieved and quality of life is promoted.
Ensure all care is delivered with dignity, respect, and a focus on client choice and independence.
Stakeholder Engagement:
Collaborate with clients, families, caregivers, and other health professionals to ensure a coordinated, person-centred approach to care.
Communicate effectively regarding client needs, progress, and care plans across service providers and care settings.
Support clients and their families to understand and actively participate in care decisions that facilitate their goals.
Administration:
Maintain accurate and timely clinical records and reports in accordance with organisational policies and legislative requirements.
Prepare reports as required by the Allied Health Lead.
Stay up to date with professional development and contribute to ongoing quality improvement initiatives.
Coordinate resources to deliver cost-effective and efficient services.
As the successful candidate, you must meet these selection criteria:
Tertiary qualification in Physiotherapy and current AHPRA registration.
Minimum 2 years' experience in Physiotherapy, with strong assessment and mobility aid prescription skills.
Excellent communication, interpersonal, and problem-solving abilities.
Ability to work independently and collaboratively as part of a multidisciplinary team.
Computer proficiency, including Microsoft Office and Client Management Systems.
Current Driver's Licence, reliable vehicle, and willingness to travel to clients in the community.
National Police Clearance (within 3 months).
