About the role

As a Social Worker in our Community Allied Health team, you will:

Provide person-centred assessment, counselling and casework to individuals (aged 16+) with chronic and complex health needs

Support clients and carers through advocacy, navigation, and connecting to services

Collaborate with interdisciplinary teams including Palliative Care and Integrated Care

Contribute to service development, student supervision and quality improvement

Deliver care across a range of settings: homes, aged care facilities, community clinics and group programs

About you

We’re seeking an experienced Social Worker who:

Holds a recognised Bachelor or Master of Social Work and is eligible for AASW membership

Have experience in health or community settings (essential for understanding local systems and contexts)

Demonstrates strong psychosocial assessment and systems navigation skills

Is compassionate, culturally safe and ethical in their practice

Works effectively within a multidisciplinary team and values collaborative care

Is committed to ongoing learning and delivering high-quality, person-centred care

What you’ll need

A current, unrestricted NSW Driver's Licence

National Criminal Record Check, Working with Children Check, and Aged Care Check
