About the program
The Aftercare service is a non-clinical support service focused on providing psychosocial support to people experiencing a suicidal crisis or who have attempted suicide. Wellways works in partnership with the community and other organisations to deliver this vital service in the Murrumbidgee region.
About the opportunity
We are seeking a compassionate and motivated Support Coordinator to join this newly created team. In this role, you will walk alongside individuals for up to three months following a suicidal crisis or attempt, offering tailored support to help them reconnect with life and community.
You will work under the guidance of the Team Leader and in collaboration with internal and external stakeholders to support the ongoing delivery of this important service.
Responsibilities will include:
- Working collaboratively with participants with a recovery or strengths-based approach to formulate, implement and review support plans, needs assessments and safety plans and to monitor progress through completion and review of outcome measures.
- Participation in intake and assessment processes with referral partners.
- Assisting participants to connect with relevant health, clinical and community-based support services
- Related administration such as (but not limited to) maintenance of client information records and activities, monitoring and evaluation activities and effective management and sharing of information with other service providers where required.
To be successful in this role, you will need:
- Relevant qualifications or commensurate experience in working in the mental health or community services sector, generally a Diploma level qualification
- An understanding of the biopsychosocial model of mental health and how social inclusion principles are applied to service delivery
- Experience working in a mental health related roles with men in rural regions, young people or children and with LGBTQIA+ communities is highly regarded
- Current valid Driver's License and the ability to undertake some travel and be away from home overnight
- Strong communication and interpersonal skills
- Right to work within Australia
- Current valid Driver's Licence and the ability to undertake some travel
- NDIS Worker Orientation Module Certificate (free online course)
- NDIS Worker Screening Check
- Satisfactory National Police Records Check
- Working with Children's Check (Employee/Paid)
