Western Queensland Primary Health Network (WQPHN) is seeking a passionate and experienced professional to join our Primary Mental Health & Wellbeing team.
This key role drives the delivery of the Western Queensland Family, Domestic & Sexual Violence (FDSV) Pilot by embedding trauma-informed, culturally safe, and multidisciplinary outreach care models throughout rural and remote communities.
About the Role
You will lead coordination of key pilot activities, foster partnerships with health and social services, and drive innovative service navigation and system integration to strengthen care pathways for FDSV victim-survivors. Your work will ensure sustainable, high-quality support that is responsive to community needs and culturally respectful, especially for Aboriginal and Torres Strait Islander peoples.
This role will have a maximum-term contract ending on 30 June 2026, with potential for ongoing employment.
Key Responsibilities
Coordinate the Department of Health, Disability & Ageing's PHN outreach pilot for FDSV victim-survivors.
Engage community leaders, service providers, and stakeholders to assess local FDSV and mental health needs.
Develop and maintain a dynamic directory of commissioned services supporting FDSV, mental health, and social care.
Co-design stepped-care mental health referral pathways specific to FDSV contexts in partnership with the Primary Mental Health & Wellbeing team.
Facilitate care coordination through the Western Queensland Connected Care and Remote Engagement (CCARE) model.
Lead capacity-building initiatives including provider training and communities of practice on trauma-informed, culturally safe care.
Oversee program monitoring, data management, and reporting on service delivery and outcomes.
Advocate for continuous improvement, innovation, and cultural safety in service commissioning and delivery.
About You
Our ideal candidate will bring experience designing collaborative referral pathways between domestic and family violence (DFV) and mental health services—particularly where this has involved co-designing service models, applying creative problem-solving, and responding to community feedback and emerging evidence in rural and remote settings.
You will also have:
Tertiary qualifications in Public Health, Community Services, Social Work, Mental Health, or a related discipline. Knowledge of stepped-care mental health models and PHN commissioning frameworks will be highly regarded.
Demonstrated experience in commissioning or contracting health, mental health, or DFV services, with a proven track record in program administration, contract management, and performance monitoring.
Demonstrated experience in working with the DFSV service system with a gendered and trauma-informed lens, as well as high level understanding of intersections of family violence, sexual violence and childhood sexual abuse with health outcomes and other psychosocial complexities.
Demonstrated knowledge and understanding of the primary health care sector, particularly healthy ageing and aged care needs of regional, remote and rural communities including Aboriginal & Torres Strait Islander Peoples.
Demonstrated understanding of trauma-informed principles and the ability to apply culturally safe approaches, particularly when working with Aboriginal and Torres Strait Islander communities and other priority populations.
Highly developed interpersonal communication skills to build and maintain internal and external relationships, with demonstrated excellence in stakeholder engagement,
Ability to work effectively with minimal supervision, manage conflicting deadlines
Competency in Microsoft Office (including SharePoint) and other office productivity tools, with aptitude to learn new software and systems.
Hold a current driver’s licence, unrestricted for work purposes, with ability and willingness to travel regularly to remote and regional communities.
