Responsibility

Supporting and empowering participants to choose how they want to lead their lives.

Coordinating various services for participants to support them in living fulfilling and meaningful lives within the community.

Engaging with and managing participants' key stakeholders (including government agencies, community, and clinical service providers and families) and ensuring that the supports and services are leveraged, not duplicated.

Working with participants strengthens their ability to coordinate support to live their best lives.

Supporting participants strengthen their capacity to establish their network and maintain support services, links, and relationships.

Supporting participants in understanding their NDIS plan, goals, and objectives whilst promoting choice and control.

Preparing for Plan Reviews and Attending with the Participant, Family, or Representative

Skills & Experience

Relevant Tertiary qualifications in Community Services, Mental Health and Disability/and or Similar

1+ years of experience in a similar role.

Demonstrated experience in facilitating multi-disciplinary coordinated planning and developing collaborative relationships and networks between service providers.

Extensive knowledge of Mainstream and NDIS systems and Practices.

Excellent verbal communication skills and a demonstrated ability to engage with participants and their families
