About the Role:
Join North Canberra Hospital as a Clinical Care Coordinator and lead a dedicated team of Discharge Liaison Nurses and Officers. In this dynamic role, you'll drive safe, efficient, and patient-centred discharges-from hospital to home or other care settings.
You'll provide expert clinical leadership, collaborate across disciplines, and work closely with internal and external partners to ensure high-quality care transitions. Your leadership will support compliance with National Safety and Quality Health Standards and foster team development and professional growth.
