About the service
In a peaceful, rural area south of Brisbane, Logan House and Logan Family Recovery based at Chambers Flat, offers a residential alcohol and drug treatment program, which is part of a continuous package of care. With the goal to encourage residents to live life well, Logan House and Logan Family Recovery offers a specialist alcohol and drug treatment residential program for individuals and parents aged 18 years and over. Our program aims to provide a safe and supportive environment free from alcohol, illicit substances, and non-prescription medication to support our resident’s treatment journey.
Your opportunity
As an Aftercare Clinician (Continuing Care) you will provide after-care support to clients from Logan House or Logan Family Recovery programs. You will be responsible for coordinating high-intensity AOD care for adults with complex needs, working closely with the Logan House and Logan Family Recovery team.
You will support individuals' transition from residential treatment to community living, supporting them throughout their stay through the facilitating on-site and community groups. The Aftercare Clinician will possess a strong understanding of clinical risk; and the ability to triage clients based on assessment.
This position is a part time contract role - 4 days per week for 12 months commencing end of July 2025, working across our Chambers Flat and Beenleigh sites.
What you’ll bring
To succeed in this role, there are some essentials you’ll need:
- You’ll be able to show us the difference you’ve made working with clients with AOD issues/concerns, applying your knowledge of contemporary evidence-based AOD counselling practice, case management, intake and assessment
- A National Criminal History Check, along with a Working with Children Check (e.g. Blue Card).
While not essential, we’d also love you to have:
- Minimum Bachelor level qualifications in a related health services discipline, which could include Social Work, Psychology, Human Services, Counselling - with additional AOD qualifications highly regarded.
