About the service

The Transitional Recovery program is designed to support adults to transition from an acute mental health setting to living in the community and to prevent further hospital admissions. In a homelike environment with 24-hour access to staff, people are supported to achieve their recovery goals, strengthen relationships with families and carers, develop social connections, and build confidence to live independently and safely in the community. The program provides personalised one-to-one support, shared supports, workshops, and group activities based on the person’s strengths and goals as identified in their individual recovery plan.

About the role

We are recruiting fixed-term, part-time Community Mental Health Practitioners to join our new Transitional Recovery program in Rockhampton. In this role, you will work collaboratively with people, families, and carers to deliver personalised, recovery-oriented care. You will support service users to develop daily living skills, foster social inclusion, and rebuild confidence for independent living. This position requires flexibility to work a 24/7 rotating roster, including night shifts, weekdays, weekends, and sleepovers.

Key responsibilities

  • Purposefully engage with people using techniques including: Brief intervention, Motivational interviewing and coaching, Family inclusive practice, Trauma informed practice, Conflict resolution.
  • Plan and develop group work programs that will assist service users to build their skills, focus on their recovery and work towards transitioning back to their natural community.
  • Support family and carer roles through understanding their concerns and the provision of information, education and referrals.
  • Collaborate with a range of services and the community to ensure a coordinated, integrated response to people’s recovery goals.
  • Contribute to the day to day operations of the service through undertaking a range of housekeeping duties.

What you’ll bring

  • Tertiary qualifications (minimum Certificate IV) in Youth Work, Mental Health, Psychology, Social Work, Occupational Therapy, or other health-related field.
  • Proven experience in working with people with mental health issues, complex needs and interacting with families and carers.
  • Proven capability to implement therapeutic, recovery oriented frameworks and practices including risk mitigation, trauma informed care and family inclusive practice.
  • Hold a current First Aid and CPR certificate.
  • Ability to provide a record of Vaccination Preventable Diseases (or able to obtain vaccinations) against Measles, Mumps, Rubella, Chicken Pox, Whooping Cough, Hepatitis B or other diseases as required by our Partnership with QLD Health.
