We’re currently seeking a Registered Nurse / Case Manager to join our team based in Camp Hill. This role is ideal for a proactive and compassionate leader who thrives in a collaborative environment and is committed to delivering outstanding care outcomes.

Key Responsibilities
Deliver holistic case management from initial referral through to service planning, implementation, and ongoing reviews
Develop and manage individual care plans and budgets in consultation with participants, carers, and families
Ensure services are implemented in line with client care plans and within NDIS and CDC frameworks
Conduct clinical reviews and provide clinical advice and training to support staff
Build strong, respectful relationships with participants, families, and support networks
Help participants set and achieve meaningful, realistic goals
Travel to clients across Brisbane, Logan, and surrounding areas as needed
About You
To be successful in this role, you’ll bring:
A nursing qualification and current AHPRA registration
Strong clinical knowledge and problem-solving ability
Experience with the NDIS and My Aged Care (Home Care Packages)
Excellent communication and customer service skills
Ability to build positive relationships with participants, families, and team members
A current, unrestricted Driver’s Licence and reliable vehicle
NDIS Support Coordination experience is a plus (but not essential)
Applicants with NDIS sector experience will be highly regarded.
