Care Coordinator role. 30 hours per week. Flexible work location.

Nurse Next Door Melbourne North is growing so we are on the hunt for a superstar Care Designer/ Registered Nurse, who will ensure that our clients are safe and happy in their own homes and lead a team of caregivers who love what they do. You may know similar roles as a Care Manager, Care Co-Ordinator or Clinical Care Manager. This role is a Part-Time role of approximately 30 hours per week, with the opportunity to increase to Full Time for the right and willing candidate. This role is ideally suited to those who have newly graduated or have been working for a few years and are looking for their next challenge.

Nurse Next Door Melbourne North, owned and operated by Greensborough locals, has been serving the community for almost two years. We are now seeking a Care Ambassador, living locally, to join our leadership team to underpin our continued growth.

Your Next Adventure

Operating in a home care environment, you'll be responsible for ensuring that our clients are safe and happy in their own homes. You'll liaise with potential clients, set up individualised care plans to meet their unique needs and build professional relationships in the health industry.

Reporting to our Managing Director, your role will be a combination of front line nursing work, management and development of our growing team of nurses and personal care assistants. This includes but will not be limited to, the following:

Part Time role - 30 hours per week

Design tailored and meaningful client care plans that foster positive relationships between our clients and the Care Team.

Meet with potential clients to consult on their options for premium home care services.

Oversee client clinical care and maintain meticulous care documentation records.

Clinical client visits

Be accountable for maintaining the high clinical standards set by Nurse Next Door.

Assist in the management and development of a growing team of caregivers and nurses.

Job Requirements

AHPRA Registration

Minimum 1 year post registration experience (desirable but not required)

CPR & First Aid Certification

Current Victorian Drivers License

Residing in Melbourne's North (We need you close by our clients)

National Police Check (Aged Care/ Vulnerable Persons Sector)

Working With Children Check

NDIS Worker Orientation Module Completion + NDIS worker screen

The nature of this role requires rotating on call evenings and weekends.
