About the role
Walk into a dynamic and impactful Occupational Therapist role, providing comprehensive assessments, interventions, and therapies to elderly clients under CHSP, HCP, and other funding programs. You'll work closely with clients to understand their individual goals, providing guidance, support, and strategies to enhance independence, safety & wellbeing.
You will also:
- Deliver high-quality & person-centred occupational therapy services, including assessments, care planning, and home modifications.
- Develop comprehensive treatment plans tailored to individual client needs & write detailed reports, including home modification assessments
- Maintain clear communication with clients, caregivers & stakeholders
- Conduct internal and external referrals to other members of the multidisciplinary team, and external funding providers including SWEP and Geat2Go.
- Essential travel across various aged care planning regions and where operationally needed.
About you
As an experienced OT, you can provide occupational therapy assessments and service delivery, including assessments and development of comprehensive plans and report writing. You have strong interpersonal skills and love engaging with a variety of stakeholders. You have strong communication skills and can maintain professional relationships.
In addition, you have:
- Tertiary Occupational Therapy qualification with current registration with AHPRA
- Current Victorian driver’s license
- Knowledge and experience of Government funded aged care programs including CHSP, HCP, STRC, SWEP Geat2GO and My Aged Care are desirable but not required.
- Ability to work autonomously and have access to formal supervision and support.
