Due to an expansion of our aged care team, Kura Yerlo is seeking to employ a qualified and experienced Aged Care Connector (ACC).
Essential Duties and Responsibilities:
The Aged Care Connector (AAC) has the primary responsibility of connecting with local Elders, older Aboriginal and Torres Strait Islander people and their families to raise awareness of their aged care entitlements.
The overarching function of the AAC is to ensure that support for clients is organised, sequenced and supported to enable effective access to appropriate aged care services both internal and external to the Aboriginal Community Controlled Health Organisations (ACCHO).
The AAC will work closely with the Aged Care Support Coordinator (ACSC). They will also help determine the level of local demand for aged care services, identify existing aged care services, and any service gaps in the area and continue advocacy work with clients as required.
- Execute active community outreach to promote aged care services, and identify and engage with potential clients.
- Support clients to identify goals and needs to ensure they get the appropriate level of care and services, which can include referrals to other specialised services and support.
- Support clients’ aged care needs through internal referral processes.
- Assist clients with advocacy and support throughout the My Aged Care registration and review processes.
- Provides support to enable client access to appointments.
- Liaises with the aged care team to continuously review and enhance the role.
Education and/or Knowledge and Experience:
- Excellent and strong relationships with the local Aboriginal and Torres Strait Islander Community
- Good communication and interpersonal skills, with the ability to build trusting relationships in community.
- Demonstrated understanding of the principles of person-centred care and trauma-informed practice.
- Understanding of Kura Yerlo services available to clients.
- Ability to or willingness to learn how to enter data and reporting.
- Demonstrated experience working unsupervised and as part of a team.
Physical Requirements:
- Ability to perform the essential job functions safely and successfully.
- Ability to maintain regular, punctual attendance.
Special Conditions:
- Current and valid South Australian Drivers (unrestricted), appropriate vehicle and full comprehensive car insurance.
- Knowledge of or willingness to learn about the aged care landscape.
- Experience working with older people.
- A qualification in community services and Aboriginal mental health first aid is desirable.
- Current HLTAID011 Provide first aid certificate highly desirable.
- Current NDIS Worker Check and National Criminal History Record Check prior to and throughout employment, to be updated five yearly.
- Attendance at out of hours meetings training and service functions may be required.
- Applicants may need to be vaccinated against COVID-19 or have an approved medical exemption.
