We’re currently expanding our multidisciplinary team and seeking a Credentialled Diabetes Educator to deliver the National Diabetes Education Program to eligible clients via Telehealth (phone and video consultations).
About the Role:
As a Credentialled Diabetes Educator, you will play a key role in delivering diabetes education and support to DVA recipients across Australia via Telehealth. Working both independently and as part of a multidisciplinary team, you will provide person-centered care that empowers individuals to manage their condition and make informed health decisions.
This remote position offers the opportunity to make a meaningful impact while supporting a valued client group through evidence-based education and compassionate care.
Key Responsibilities:
Diabetes Education: Provide education on nutrition, medication management, blood glucose monitoring and lifestyle modifications.
Person-directed care: Create person-centred, taking into account each individual’s unique goals, preferences and literacy.
Telehealth Consultations: Conduct individualised telehealth consultations with DVA recipients for assessment, completion of a patient care plan and implementation of The National Diabetes Education Program.
Multidisciplinary Co-ordination: Refer clients to the Allied Health team, and to Case Conferencing (with GP and multidisciplinary team), as needed.
Empowerment: Practise evidence-based health coaching techniques to empower clients to take control of their health by delivering content, providing guidance on self-management and decision-making to achieve positive health outcomes.
Documentation: Maintain accurate and detailed patient records.
Stay Informed: Stay up-to-date with the latest advancements in diabetes management, treatment options and educational resources to continue to
contribute to the development of The National Diabetes Education Program.
Compliance: Ensure diabetes educator services align with DVA guidelines, and relevant professional standards and codes of conduct.

Requirements:
Current ADEA registration as a Credentialled Diabetes Educator
Current certification with a practising professional body (E.g. AHPRA, ESSA, DAA)
Excellent communication skills and ability to establish and maintain therapeutic client relationships
Ability to deliver diabetes education and support via telehealth
Ability to work independently and remotely as part of a national network of multidisciplinary services
