About the role
The Therapeutic Case Manager provides specialist casework and therapeutic intervention for children and young people, their families, carers, within the permanency support program using contemporary practices.
The key focus area of the role will be:

Carers
You will be responsible for:
Develop and implement case plans for children and young people in collaboration with relevant stakeholders and provide ongoing therapeutic intervention and case management.
Engage with children, young people, families and carers, and undertake assessments to identify their support needs.
Assist in the transition of children and young people entering and exiting permanency support program care arrangements.
Maintain accurate records and documentation, in line with the office of the children’s guardians practice standards, which may be used for the purposes of children’s court proceedings.
Network with other relevant stakeholders, as required.
About you
To be a successful candidate, you will have:
Tertiary Qualifications degree in teaching, Social Science, Nursing, Counselling.
Experience working with and providing quality assessment and case management services to at risk families or children/young people in out-of-home care and carers for a Minimum of 5 years.
Knowledge and experience to work within the relevant Child Protection legislation in NSW.
Demonstrated experience in case load management of 10 – 12 CYP, inclusive of monthly home visits, 1:4 Family Time visits, ability to develop and review case plans, ability to develop cultural plans, and understanding on how to review Positive Behaviour Support Plans.
CDP Certificate from DCJ, and / or Knowledge of the Child Protection legislation that underpins the work of a case manager in Out of Home Care including knowledge of the Office of the Children’s Guardian NSW Child Safe Standards for Permanent Care and the Children & Young Person (Care and Protection Act) 1998.
Demonstrated experience working with CALD and Aboriginal and Torres Strait clients
Commitment to working within a diverse and inclusive environment
Current Working with Children Check Clearance
Nationally Coordinated Criminal History Check (NCCHC)
Current and valid Driver’s Licence with access to a well-maintained vehicle which will be used for work purposes
