Case Manager Senior Relationship Service

Your new role

Part time (0.6 FTE) Fixed Term 12 Months based in Wagga Wagga, NSW

Annual salary between $98,185.54 to $ 102,491.77 plus Superannuation, NFP Salary Packaging, Paid Christmas closure and other generous leave provisions under our Enterprise Agreement.

The case manager strengthens the well-being of older adults experiencing, or at risk of experiencing abuse. The case manager provides client focused and trauma informed support to coordinate access to services.

Duties

Under the general direction of the Team Leader, the Case Manager is responsible for:

Completing psychosocial assessments and screening for risk and implementing client-centred case plans.

Providing information to clients and their families on government and community resources for practical and financial support. Coordinate access to services required by the client and their family in alignment with their case plan.

Working collaboratively with the RACR clinical team and external stakeholders to access mediation, counselling, legal and other support for clients.

Conducting risk assessments, developing safety plans, and providing crisis intervention as necessary.

Working in partnership with and engaging local community stakeholders and networks to raise awareness of the program and inspiring commitment to eliminating elder abuse.

What you’ll need to succeed

Relevant experience and qualification (or working towards a qualification) in the field of Social Work, Psychology, Nursing, Occupational Therapy, Counselling, or Social Science. Current registration/membership with at least one of the following: Australian Association for Social Workers (AASW); Australian Health Practitioners Regulation Agency (AHPRA); Psychotherapy and Counselling Federation of Australia (PACFA) and Level 2 or above Australian Counselling Association (ACA).

Demonstrated skills and experience in providing case management and an in-depth understanding of elder abuse, ageing, and the impact of FDSV on families.

Demonstrated knowledge of theoretical frameworks for providing client centred and strengths-based intervention that is trauma-informed, and crisis intervention.

Demonstrated skill in developing and maintaining collaborative relationships with relevant internal and external stakeholders.

Demonstrated experience working with diverse cultural communities, and knowledge of issues facing Aboriginal and Torres Strait Islander people, people with a disability, culturally and linguistically diverse individuals and communities, and LGBTQIA+ individuals and communities.
