The Salary range is currently $51.75 to $53.84 per hour. Hourly rate based on experience.

Location: Fair Haven Aged Care is located in a semi-rural part of Hervey Bay, on 40 beautiful acres offering cool breezes, walking paths and abundant birdlife. What You’ll Do

As Care Coordinator, you’ll lead the delivery of high-quality clinical care for residents with complex needs, while working a hands-on roster that includes PM shifts on weekdays and AM shifts on weekends. This structure ensures strong clinical oversight during key after-hours periods and weekends.

In this pivotal leadership role, you will:

• Oversee resident assessments and develop tailored care plans
• Coordinate clinical documentation and AN-ACC compliance reporting
• Provide direct, on-the-floor clinical support and leadership
• Mentor and support a team of RNs, care staff, and volunteers
• Liaise with external providers to ensure seamless care delivery
• Monitor and enhance clinical programs and quality indicators
• Foster a culture of safety, wellbeing, and continuous improvement
  
  

About You

As a Care Coordinator, you’ll play a pivotal role in leading clinical care delivery for residents with complex needs. You’ll oversee assessments, care planning, and compliance, while mentoring a team of Registered Nurses, care staff, and volunteers. Your leadership will ensure our residents receive holistic, person-centred care aligned with our Positive Wellbeing Model.

This is a key leadership position reporting to the Service Manager, with direct responsibility for clinical staff and indirect influence across the broader care team.

Requirements:

• AHPRA Registration as a Registered Nurse (RN) with a current Practicing Certificate

• Minimum 3 years’ post-graduate experience in aged care or acute clinical settings
• Proven leadership and workforce management skills
• Strong clinical assessment, care planning, and documentation capabilities
• Intermediate computer literacy (Word, Excel, Outlook, PowerPoint)
• A commitment to quality, safety, and continuous improvement
• Current National Police Check – or willingness to acquire
• Current First Aid & CPR certification – or willingness to acquire
• A Valid driver’s licence
  
  

Desirable:

• Postgraduate qualifications in Gerontology, Dementia, or Aged Care
• Experience with mobile clinical documentation systems
