Northern Health are seeking applications from motivated pharmacists to join the Pharmacy Department as Grade 2 Registrar Training Pharmacist - Transitions of Care
Northern Health is the major provider of acute, maternity, sub-acute, mental health, specialist, community and home-based services in Melbourne's rapidly growing outer north.
Services are provided through our five main campuses: Northern Hospital Epping, Broadmeadows Hospital, Bundoora Centre, Craigieburn Centre, and Kilmore District Hospital. In addition, Mental Health Services are provided at Epping, Broadmeadows, Jacana, Preston, Mill Park and Coburg.
About the Pharmacy Department:
The Pharmacy Department operates across five campuses and has a strong vision to deliver exceptional, evidenced-based care to our patients - with a clear focus on research, medication safety and clinical excellence. Meeting the immediate and longer-term health care needs of our community requires a transformation in how we provide health care services, and Northern Health has embarked on the journey to transform into a High Reliability Healthcare Organisation. The department comprises over 100 pharmacists and 30 technicians to provide clinical, operational, quality and informatics services with a strong focus on workforce culture and commitment to professional development.
We are currently undergoing a period of expansion which provides unique opportunities for specialisation appealing to those applicants wishing to challenge themselves and advance practice including pharmacist and technician advanced-scope roles, collaborative multidisciplinary programs and services, medicine stewardship programs (e.g. analgesics, anticoagulants and antimicrobials) and virtual care (including Heart Failure Virtual Ward and Victorian Virtual Emergency Department).
About the Role:
An opportunity exists for a Grade 2 Registrar Training Pharmacist - Transitions of Care and become part of the inaugural AdPha-accredited Transitions of Care program in Australia.
The Registrar Training Pharmacist will be supported to advance their practice in Transitions of Care and develop their skills through completion of a two-year program as an AdPha Registrar Candidate in Transitions of Care. The pharmacist's work plan will include 18 months in transitions of care services (including Hospital In The Home, Heart Failure Virtual Ward, Complex Care and Transitions of Care) and 6 months in a relevant specialty area (eg. inpatient General Medicine) under the mentorship of the Senior Pharmacist - Hospital in the Home and Senior Pharmacist - Transitions of Care. They will continue as a rotational clinical pharmacist within the Pharmacy Department following completion of the two-year program.
Northern Health's Hospital Without Walls Division provides a broad range of programs to support the transition from hospital to home, prevent the need for hospital presentations and helps manage acute and chronic conditions that require specialist care. These include Hospital in the Home, Residential In Reach, Post-Acute Care and HARP. Northern Health's Pharmacy Transitions of Care Service provides post discharge follow up care via telehealth focusing on priority issues outlined in discharge summaries.
To be successful in this role you will have:
- A minimum of 3-5 years full time experience as a hospital pharmacist
- A demonstrated passion for working in Transitions of Care
- Experience and good working knowledge of hospital pharmacy operations.
- Demonstrated good interpersonal and communication skills.
- Well-developedself-management skills including the ability to meet timelines and prioritise competing demands
