About the Opportunity
We currently have an exciting opportunity to join the Metropolitan Youth Health (MYH) team as a Community Nurse, where you will be responsible for:
- Promote and implement a client-centred care approach within a case management framework, focusing on young people's health and well-being.
- Develop and document comprehensive case work services, including assessments, health screenings, treatment, referrals, and psychosocial support, in line with the MYH Model of Care.
- Provide expert social support, advocacy, and referral services to vulnerable young people, ensuring they have access to appropriate primary healthcare resources.
- Design and deliver projects aimed at improving young people's health literacy and empowering them to manage their healthcare needs.
- Collaborate with interagency networks, evaluate service delivery, and contribute to continuous quality improvement efforts, while maintaining clinical documentation in accordance with organizational policies.
About You
To be successful in this role, you will have:
- Registered or eligible for registration as a Nurse by the Nursing and Midwifery Board of Australia (NMBA) and who holds, or who is eligible to hold, a current practicing certificate.
- Registered Nurse and Midwife with at least 3 years post registration experience.
- Well-developed interpersonal and communication skills including the ability to work with individuals and families for positive health outcomes and to foster productive working relationships across disciplines.
- Demonstrated ability to display initiative, flexibility and creativity in response to the changing needs of the clients and the organisation.
- Knowledge of contemporary nursing evidence based practice issues, primary health care practice and continuous improvement principles.
