The McGrath Foundation is a registered charity providing care and support to all people with cancer. The Foundation provides resources and funding to allow the engagement of appropriately qualified Cancer Care Nurses, servicing hospitals, health care services and communities throughout Australia. The All-Cancer Nurse Service is part of the Commonwealth Government’s – Australian Cancer Nursing and Navigation Program, a response from the Commonwealth Government to the Australian Cancer Plan launched by Cancer Australia. Northern NSWLHD has been funded by McGrath Foundation for 2 part-time, temporary positions for the next two years with one each at Tweed Valley Hospital and Lismore Base Hospital’s cancer services. The Tweed Vally Hospital’s role will have a pan-tumour cover looking at highly complex patients with high needs for care coordination (exclusive of breast, paediatrics and haematology). The complexity would be defined by having multi-modality anti-cancer therapy with time urgency. Patients will be referred from an existing MDT meeting. Complex surgery is often referred to Queensland facilities so the Tweed CCC would have high engagement with the tumor stream specific CNC’s at those facilities to support the patient returning for concurrent/adjuvant therapies closer to their home and social supports. Care coordinators will utilise the national Optimal Care Pathways for people with cancer and existing local referral processes within and between Northern NSW Local Health District facilities and other entities.

About the role

In this role the Clinical Nurse Consultant Grade 2 (CNC2) McGrath All Cancer Care Coordinator is required to provide an advanced level nursing service as an expert in the specialty field and facilitates linkages and relationships with Local Health District nurses/midwives and medical officers to support patient management. Providing assessment, care planning and review of patients in collaboration with the primary service providers to ensure that the McGrath Model of Care and other certified evidence-based practices are utilised. The Clinical Nurse Consultant (CNC2) is responsible for expert clinical coordination, leadership and consultancy services within the oncology service and target populations within the Northern NSW Local Health District (NNSWLHD). The current McGrath All Cancer Care Coordinator’s scope of practice is care coordination of patients receiving concurrent Chemoradiation therapy anti-cancer treatment in all NNSWLHD cancer centres.
