About the Role

Assess, diagnose, and manage a wide range of health conditions in the community

Provide health education, promote wellness, and help prevent disease

Collaborate with multidisciplinary teams to develop holistic care plans

Support the design and delivery of local community health programs

Contribute to improving and evaluating community health services

What You’ll Bring

Current Nurse Practitioner registration with Ahpra (or will be eligible to apply)

Strong clinical assessment, diagnostic, and management skills

Significant experience in community or primary healthcare

Ability to work both independently and within a team

Excellent communication, collaboration, and patient education skills

A commitment to evidence-based care and ongoing professional development
